It's from SGS. From the 1975/76 manual "Consumer Trasistors & ICs":
7 W Audio power amplifier with thermal shut-down. Intented for use as 
low freq. class B amplifier. 7W@16V-4Ohm, 6W@14.4V-4Ohm, 2.5W@9V-
4Ohm, 1W@6V-4Ohm. Up to 20 V power supply, up to 2.5 A output 
current. Short circuit protected.
1-Supply, 2-nc, 3-nc, GROUND, 4-bootstrap, 5-compensation, 6-
feedback, 7-ripple rejection, 8-input, 9-ground (substrate), GROUND, 
10-Ground, 11-nc, 12-output.
